Edwards Dave Mathews Scholarship Award
 

General Information

The award recipient will receive a $200.00 award and commemorative plaque. This award highlights a commitment to occupational health excellence by Edwards Medical Supply.

All applications will be reviewed and approved by the CSAOHN Awards Committee. The Awards Chair will notify the individual that submitted the nomination for consideration of its receipt within two weeks. The winning recipient will be notified and recognized at the annual CSAOHN business meeting, which will be held in the fall at the State Conference.

Candidate Criteria

1. Three (3) years as a California Registered Nurse with a minimum of one year (1) in the field of Occupational Health Nursing at the time of application.
2. Active, Associate or Student membership in AAOHN/CSAOHN for the current year and past year.
3. Indicate how the scholarship will be used, specifically listing conference titles, continuing education units, course work, etc.
4. Provide a current resume which includes current and past leadership activities
5. Submit a one-page statement of professional and educational goals to include:
   - How the financial award obtained will be used to enhance your skills as an occupational health nurse leader
   - Illustrate creativity and commitment to leadership with specific examples
   - Describe what motivates you to seek leadership opportunities
